Four easy steps.

When you or a loved one need some extra support at home, it can be hard to know where to start. For many people, Home Care Services are the solution. The process of applying for services can be broken down into four easy steps.

Our friendly bilingual staff will be there for you every step of the way. We will take the time to listen and get to know you, so we can understand your needs. We will be there to support and guide you through each step, help you understand what services might be available to you and answer any questions you may have.

Step 1 – Contact Co.As.It.

 

The first step is to contact us and tell us a bit about yourself.

We will ask you about your health, your needs and goals. This will give us a good idea of how we can support you and which of our services will best suit you.

We will talk to you about; the services in your area, your culture and language options, our staff, and costs and fees.

We will then help you to login into My Aged Care to find out if you’re eligible for government-funded services. My Aged Care is the government platform for Aged Care Services.

If you are not eligible for government-funded care, we can organise Private Home Care Services for you.

Step 2 – Get Assessed

 

We can make a referral for a free face-to-face assessment on your behalf and help you prepare and plan for your assessment. Assessments are done in person, at your own home.

There are two types of assessments; Home support assessment with a Regional Assessment Service (RAS) and Comprehensive assessment with the Aged Care Assessment Team (ACAT). The type of assessment depends on the level of support you require and the associated funding option.

The purpose of the assessment is to establish what support you might need to remain living independently in your own home. Together, you and your assessor will evaluate your eligibility and look at the range of services which could assist you.

During the assessment you can let My Aged Care know that Co.As.It. is your preferred service provider. The benefit of doing this is that when a package becomes available, we will also be informed and can contact you to start organising your care plan.

In the meantime, while waiting for the outcome of the assessment, we can organise any necessary support services for you with private funding.

My Aged Care will contact you to let you know the outcome of your assessment. They will specify whether you are eligible to receive Commonwealth Home Support Programme (CHSP) or a Home Care Package (HCP), and if so, the level (1-4) you have been approved for to meet your needs.

Step 3 – Arrange your Service

 

Once you have been allocated funding we can then help you take the next steps to arrange your services with Co.As.It.

Commonwealth Home Support Programme (CHSP)

If you have allocated funding via CHSP, services can commence within a couple of months. Our friendly team will assist you to create a care plan that is tailored to your needs and goals. Once you are happy with your quote, it is time to accept the agreement.

Home Care Package (HCP)

If you have been allocated funding via HCP, services can take between 6 months to a year. You will be placed on a national waitlist and notified when your package becomes available to you. In the meantime, we can organise any necessary support services for you with private funding.

Three months prior to receiving access to your package, you will get a ‘Readiness letter’ from My Aged Care. This is an important letter as it will contain some paperwork you will need to complete. Call us when you get this letter and we can help you complete the paperwork. At this time we can also start planning your support, so you are ready to receive services when your package becomes available.

Once your Home Care Package becomes available, you will receive a letter confirming the details of the package assigned to you. Contact us when you get this letter. Our friendly team will assist you to create a care plan that is tailored to your needs and goals. Once you are happy with your quote, it is time to accept the agreement.

Step 4 – Get the most from your service

 

Once your care plan has been set, you can relax and start using and benefiting from your services.

We’ll support you in the ongoing management of your services. You will receive regular statements and/or pay invoices. Tell us if you need to take a break (for example, go to hospital). We’ll review your services regularly to ensure your needs continue to be met.
