The Co.As.It. Home Care Services can assist you to remain independent in your own home for longer with our wide range of tailored support services. We offer services that are sensitive to cultural, linguistic and clinical care needs. Our home care services can provide a little or a lot of help to support you to live independently at home.
We provide these services throughout Sydney. The services available can vary based on where you live and whether you are accessing government funding.
Step 1 Contact Co.As.It.:
Get in touch via website, phone, or email. We’ll set up a meeting with a Case Manager to discuss your needs and how we can help you stay independent at home.
Step 2 Call My Aged Care:
Call 1800 200 422 or visit their website to inform them of your decision to transfer to Co.As.It. They’ll reactivate your referral code and send it to us. Let us know the code once you get it.
Step 3 Inform Your Current Provider:
Notify your current provider about your decision to switch. Check your agreement for notice requirements. Your Case Manager can assist with this if needed.
Step 5 Switch to Co.As.It.
