If you are considering a change, transferring to Co.As.It. as your preferred home care package provider is as simple as a few steps. Here’s how you can make the transfer:
- Select a provider that meets your needs. At Co.As.It., we pride ourselves in offering home care package services that meet a wide range of requirements, including domestic assistance, dementia-specific care, social support and culturally and linguistically diverse needs. Whether it be light cleaning and transport, in-home nursing, a dementia-specific social group, or visits from a bilingual volunteer, Co.As.It. is here to help. - Contact My Aged Care. Call My Aged Care at 1800 200 422 to reactivate your referral code and provide it to your new provider. At Co.As.It., we can provide hands on support with contacting My Aged Care, if this support is needed.
- Notify your current provider. Simply inform your current package provider around your decision to switch. Our friendly case managers at Co.As.It. can also assist with this step and help check any requirements, such as a notice period.
- Set a new start date. Once your existing package agreement ends, let us know when you would like to begin services.
